Informations cliniques

Les collagénoses sont des maladies rhumatismales. Il s’agit par exemple du lupus érythémateux disséminé, de la sclérodermie systémique, du syndrome de Sjögren, des syndromes de myosite ou du syndrome de Sharp.

Les auto-anticorps dirigés contre des composants cellulaires, généralement appelés anticorps anti-nucléaires (ANA), constituent un marqueur sérologique important des collagénoses. Les antigènes cibles comprennent par exemple les acides nucléiques, les protéines du noyau cellulaire et les ribonucléoprotéines, mais aussi des composants du cytoplasme.

La fréquence (prévalence) des anticorps anti-nucléaires dans les maladies rhumatismales inflammatoires se situe entre 20 % et 100 %. Par conséquent, un diagnostic différencié des ANA est indispensable pour identifier les différentes maladies rhumatismales et les distinguer des autres maladies auto-immunes.

Diagnostic

La méthode de référence pour la détection des ANA est le test d’immunofluorescence indirecte (IFI) avec des cellules épithéliales humaines (HEp-2), connu pour sa grande sensibilité et spécificité. L’évaluation au microscope permet de déterminer avec précision comment un colorant indicateur (généralement la fluorescéine) se répartit dans les cellules. En fonction de la localisation des différents autoantigènes, chaque type d’auto-anticorps lié donne un aspect de fluorescence typique.

Le consensus international révisé sur une nomenclature standardisée des aspects des cellules HEp-2 en immunofluorescence indirecte (ICAP, www.anapatterns.org) définit 15 aspects nucléaires, 9 aspects cytoplasmiques et 5 aspects mitotiques pertinents pour le diagnostic de diverses maladies auto-immunes.

En outre, l’ICAP recommande d’effectuer d’autres tests d’anticorps spécifiques (par exemple ELISA, immunoblot, ChLIA) pour confirmer les résultats en IFI. La seule utilisation de ces méthodes de test monospécifiques ne suffit cependant pas pour déterminer les auto-anticorps anti-noyaux cellulaires, car tous les antigènes pertinents ne sont pas encore disponibles sous forme purifiée et les ANA correspondants ne peuvent donc être detectés que par IFI.
